Наибольшую эффективность в лечении ожирения доказали методы бариатрической хирургии. Повторный набор массы тела (ПНМТ) часто встречается после различных видов бариатрических операций. Клиническая значимость ПНМТ четко не определена.</p></sec><sec><title>Цель</title><p>Цель. Оценить динамику массы тела и определить величину клинически значимого повторного набора массы тела на основании изучения показателей углеводного обмена и артериального давления у пациентов с ожирением после выполнения рукавной гастропластики (РГ) и гастрошунтирования (ГШ).</p></sec><sec><title>Методы</title><p>Методы. 68 пациентов с ожирением после РГ (n = 40) и ГШ (n = 28) наблюдались в течение 3 лет. Оценивались индекс массы тела (ИМТ), процент потерянного избыточного ИМТ (%EBMIL), ПНМТ, гликированный гемоглобин, артериальное давление.</p></sec><sec><title>Результаты</title><p>Результаты. Выявлена сравнимая эффективность РГ и ГШ по проценту потерянного избыточного ИМТ (%EBMIL) в точке надир. %EBMIL более 50% был достигнут у 78,8% пациентов после РГ и 80,0% пациентов после ГШ. ПНМТ более 15% был ассоциирован со значимым увеличением систолического артериального давления. Клинически значимый ПНМТ в группе РГ был выявлен у 32,5% пациентов, в группе ГШ – у 17,2% пациентов (р &gt; 0,05).</p></sec><sec><title>Заключение</title><p>Заключение. Повторный набор массы тела более 15% был ассоциирован со значимым повышением систолического артериального давления у пациентов с ожирением после РГ и ГШ, что позволяет считать ПНМТ более 15% клинически значимым. Клинически значимый ПНМТ через 36 мес после оперативного вмешательства выявлялся у 32,5% пациентов после РГ и 17,2% пациентов после ГШ.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background: Bariatric surgery methods have proven to be most effective in treating obesity. Weight regain (WR) is often found after various types of bariatric surgery. The clinical significance of WR is not clearly defined.</p></sec><sec><title>Aims</title><p>Aims: to assess the dynamics of body weight and determine the amount of clinically significant WR based on the study of carbohydrate metabolism and blood pressure in patients with obesity after performing sleeve gastrectomy (SG) and gastric bypass (GB).</p></sec><sec><title>Materials and methods</title><p>Materials and methods: 68 patients with obesity after SG (40) and GB (28) were observed for 3 years. Body mass index (BMI), percentage of excess BMI lost (% EBMIL), WR, glycated hemoglobin, blood pressure were evaluated.</p></sec><sec><title>Results</title><p>Results: A comparable efficiency (EBMIL more than 50%) of the SG and the GB was at the nadir point. % EBMIL over 50% was achieved in 78.8% of patients after SG and 80.0% of patients after GB. WR more than 15% was associated with a significant increase in systolic blood pressure. Clinically significant WR in the SG group was detected in 32.5% of patients, in the GB group – in 17.2% of patients (p &gt; 0.05).</p></sec><sec><title>Conclusion</title><p>Conclusion: WR of more than 15% was associated with a significant increase in systolic blood pressure in patients with obesity after SG and GB, which makes it possible to consider WR of more than 15% clinically significant.
