Ежегодно в Российской Федерации выполняется более 30000 хирургических вмешательств на щитовидной железе. Хирургами разрабатываются методы профилактики послеоперационных осложнений, в связи с чем операции на данном органе считаются относительно безопасными. В настоящее время не существует однозначной эффективной методики, позволяющей предотвратить послеоперационный гипопаратиреоз. Данное осложнение часто регистрируется и может угрожать жизни и здоровью пациента, что противоречит концепции безопасной хирургии.</p></sec><sec><title>Цель</title><p>Цель. Оценить эффективность интраоперационной ICG-ангиографии и интратиреоидного введения бриллиантового зеленого для профилактики послеоперационного гипопаратиреоза.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Было выполнено 143 тиреоидэктомий. Пациенты разделены на 3 группы: интраоперационная ангиография использовалась в 24 случаях, в 58 случаях с целью идентификации околощитовидных желез вводился бриллиантовый зеленый, в 61 случае использовалась визуальная оценка сохранности околощитовидных желез. Уровень кальция измерялся всем вошедшим в исследование пациентам до и после операции.</p></sec><sec><title>Результаты</title><p>Результаты. Уровни кальция в сыворотки крови в до- и послеоперационном периоде: 2,37±0,14 и 2,27±0,17 в группе 1, и 2,38±0,16 и 2,21±0,16 в группе 2, 2,39±0,17 и 2,18±0,19 в группе 3. Гипокальциемия в послеоперационном периоде была значимо выше в группе с визуальной оценкой околощитовидных желез относительно первых двух групп. Различия между уровнями кальция в послеоперационном периоде во 1 и 3 группе были достоверны с вероятностью более 99% (p&lt;0,05). Уровни паратгормона в до- и послеоперационном периоде составили соответственно: 8,6±0,56 и 6,2±0,4 в первой группе, во второй группе 8±0,43 и 5,6±0,57 и в третьей — 8,2±0,41 и 3,5±0,32. Различия между уровнями паратгормона были достоверны после тиреоидэктомии в 1 и 3 группе с вероятностью более 99% (p&lt;0,01) и с вероятностью ≥95% паратгормон различался в послеоперационном периоде между группами 2 и 3 (p&lt;0,05).</p></sec><sec><title>Заключение</title><p>Заключение. ICG-ангиография и интратиреоидное введение бриллиантового зеленого безопасные методы идентификации и сохранения околощитовидных желез. Высокий уровень гипокальциемии и гипопаратиреоидинемии в группе 3 показывает необходимость поиска новых методик в эндокринную хирургию с целью улучшения безопасности пациентов, подвергшихся хирургическому лечению тиреоидной патологии.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>BACKGROUND</title><p>BACKGROUND: More than 30,000 surgical interventions on the thyroid gland are performed annually in the Russian Federation. Surgeons are developing methods for the prevention of various postoperative complications, and therefore operations on this organ are considered relatively safe. Currently, there is no unequivocal effective method to prevent postoperative hypoparathyroidism. This complication is often recorded and can threaten the life and health of the patient, which is contrary to the concept of safe surgery.</p></sec><sec><title>AIM</title><p>AIM: To evaluate the effectiveness of intraoperative ICG angiography and intrathyroidal injection of brilliant green for the prevention of postoperative hypoparathyroidism.</p></sec><sec><title>MATERIALS AND METHODS</title><p>MATERIALS AND METHODS: 143 thyroidectomies were performed. The patients were divided into 3 groups: intraoperative angiography was used in 24 cases, brilliant green was injected to identify the parathyroid glands in 58 cases, visual assessment of the preservation of the parathyroid glands was used in 61 case. Calcium levels were measured in all patients included in the study before and after surgery.</p></sec><sec><title>RESULTS</title><p>RESULTS: Serum calcium levels in the pre- and postoperative period: 2.37±0.14 and 2.27±0.17 in group 1, and 2.38±0.16 and 2.21±0.16 in group 2, 2.39±0.17 and 2.18±0.19 in group 3. Hypocalcemia in the postoperative period was significantly higher in the group with a visual assessment of the parathyroid glands relative to the first two groups. Differences between calcium levels in the postoperative period in groups 2 and 3 were significant with a probability of more than 99% (p&lt;0.01). Significant differences (≥95%) in calcium levels in the postoperative period between groups 1 and 2 (p &lt; 0.05) were obtained. Serum PTH levels in the postoperative period: 6,2±0,4 in group 1, 5,6±0,57 in group 2, 3,5±0,32 in group 3. Differences between PTH levels in the postoperative period in groups 1 and 3 were significant with a probability of more than 99% (p&lt;0.01). Significant differences (≥95%) in PTH levels in the postoperative period between groups 2 and 3 (p &lt; 0.05) were obtained.</p></sec><sec><title>CONCLUSION</title><p>CONCLUSION: ICG angiography and brilliant green intrathyroidal injection are safe methods for identifying and preserving the parathyroid glands.
