Первичный гиперпаратиреоз (ПГПТ) является редкой патологией у пациентов детского и подросткового возраста. Сбор, анализ и обобщение данных литературы и опыта ведущих клиник позволяет выработать единые, статистически обоснованные подходы к диагностике и хирургическому лечению данной группы больных.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В статье выполнен ретроспективный анализ 17 наблюдений ПГПТ у детей и подростков в возрасте от 6 до 18 лет, оперированных в ФГБОУ ВО СПбГПМУ в период с 1973 по 2021 год. Среди оперированных было 10 девочек и 7 мальчиков, соотношение м:ж составило 1:1,4. Средний возраст больных составил 12,9±0,71 лет.</p></sec><sec><title>Результаты и обсуждение</title><p>Результаты и обсуждение. Основными критериями диагностики заболевания являлись повышение уровня кальция и паратгормона крови, избыточная секреция кальция с мочой. У 10 (58,8%) из 17 детей диагностированы манифестные формы заболевания. В 3 (17,6%) наблюдениях новообразования околощитовидных желез (ОЩЖ) не сопровождались ни клиническими, ни лабораторными проявлениями заболевания, и расценены как инциденталомы. Еще у четырех (23,5%) пациентов имелись только лабораторные изменения (гиперкальциемия и гиперпаратиринемия). Эти наблюдения отнесены к бессимптомной форме ПГПТ.</p><p>В 7 (41,2%) случаях паратиреоаденомы обнаружены и удалены в ходе операций по поводу различных заболеваний щитовидной железы (в 5 наблюдениях — тиреоидный рак, в 1 — диффузный токсический зоб). Случайной находкой паратиреоаденома стала в ходе профилактической тиреоидэктомии по поводу синдрома Сиппла.</p><p>Наиболее информативным способом локализации паратиреоидных опухолей оказался радиоизотопный метод.</p></sec><sec><title>Заключение</title><p>Заключение. Диагностика заболевания при манифестных спорадических случаях не отличается от таковой у взрослых. При наличии семейного анамнеза, множественного поражения ОЩЖ показано генетическое исследование. Хирургическое лечение в объеме удаления паратиреоидной опухоли — основной метод, позволяющий добиться полного выздоровления.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Primary hyperparathyroidism (PHPT) is a rare pathology in pediatric and adolescent patients. Collection, analysis and generalization of the literature data and experience of the leading clinics allow to develop unified, statistically substantiated approaches to diagnostics and surgical treatment of this group of patients.</p></sec><sec><title>Material and methods</title><p>Material and methods. The article presents a retrospective analysis of 17 cases of PHPT in children and adolescents aged from 6 to 18 years operated on in the department of Surgery of St.-Petersburg State Pediatric Medical University in the period from 1973 till 2021. Among those operated there were 10 girls and 7 boys, the M:F ratio was 1:1.4. The mean age of the patients was 12,9±0,71 years.</p></sec><sec><title>Results and discussion</title><p>Results and discussion. The main criteria of the disease diagnosis were elevated blood calcium and parathormone levels, excessive urinary calcium secretion. Manifest forms of the disease were diagnosed in 10 (58,8%) of 17 children. In 3 (17,6%) cases the parathyroid neoplasms were accompanied neither by clinical, nor laboratory manifestations of the disease and were regarded as incidentalomas. Another 4 (23.5%) patients had only laboratory changes (hypercalcemia and hyperparathyrinaemia) that manifested themselves preoperatively. These observations were referred to the asymptomatic form of PHPT.</p></sec><sec><title>In 7 (41</title><p>In 7 (41.2%) cases parathyroid adenomas were found and removed during surgeries for various thyroid diseases (thyroid cancer in 5 cases, diffuse toxic goiter in 1 case). An incidental finding of parathyroid adenoma was during prophylactic thyroidectomy for Sipple syndrome.</p><p>A radioisotope method proved to be the most informative way to localize parathyroid tumors.</p></sec><sec><title>Conclusion</title><p>Conclusion. The diagnosis of the disease in manifest sporadic cases does not differ from that in adults. Genetic study is indicated in the presence of a family history of multiple involvement of the parathyroid glands.
