Одной из важнейших задач при проведении операций на щитовидной железе является предотвращение развития специфических осложнений — нарушения функции гортанных нервов и гипопаратиреоза. На решение данной задачи направлено значительное число технических решений, внедренных в клиническую практику в последние годы.</p></sec><sec><title>ЦЕЛЬ</title><p>ЦЕЛЬ. Оценка результатов применения альтернативной методики проведения операции на щитовидной железе — медиальной тиреоидэктомии.</p></sec><sec><title>МАТЕРИАЛЫ И МЕТОДЫ</title><p>МАТЕРИАЛЫ И МЕТОДЫ. Были прооперированы 270 пациентов с заболеваниями щитовидной железы с использованием методики TFT в объеме геми- или тиреоидэктомии с или без шейной лимфаденэктомии. Выборка пациентов была сплошной. Всем пациентам до операции и в 1-е послеоперационные сутки производилась УЗ- либо видеоларингоскопия. Пациентам, перенесшим полное удаление ткани щитовидной железы, выполнялись анализ крови на паратгормон и ионизированный кальций.</p></sec><sec><title>РЕЗУЛЬТАТЫ</title><p>РЕЗУЛЬТАТЫ. Для гемитиреоидэктомии длительность операций не изменилась. В группе пациентов с узлами размером до 30 мм длительность операции увеличилась. Также увеличение времени операции отмечено при выполнении тиреоидэктомии. Односторонний парез гортани зарегистрирован у 6 (2,3%) пациентов, или 1,7% числа возвратных гортанных нервов в зоне риска. Риск удаления околощитовидной железы (ОЩЖ) составил 0,39% числа операций и 0,14% числа ОЩЖ в зоне риска. Снижение уровня паратгормона в 1-е сутки после операции отмечено у 11,5% пациентов. Развитие послеоперационной гематомы было отмечено у 1 пациентки (0,39%).</p></sec><sec><title>ЗАКЛЮЧЕНИЕ</title><p>ЗАКЛЮЧЕНИЕ. Методика проведения операции на щитовидной железе с медиальным доступом к возвратному гортанному нерву и ОЩЖ выполнима у подавляющего большинства пациентов с заболеваниями щитовидной железы, требующими хирургического лечения, и показывает высокую безопасность и ряд преимуществ перед традиционным методом выполнения операций на щитовидной железе.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>BACKGROUND</title><p>BACKGROUND: One of the most important tasks in thyroid surgery is to prevent the development of specific complications — laryngeal nerve dysfunction and hypoparathyroidism. A significant number of technical solutions introduced into clinical practice in recent years are aimed at solving this problem.</p></sec><sec><title>AIM</title><p>AIM: Тo evaluate the results of an alternative technique of thyroid surgery — medial thyroidectomy.</p></sec><sec><title>MATERIALS AND METHODS</title><p>MATERIALS AND METHODS: 270 patients with thyroid diseases were operated on using tension free thyroidectomy technique in the volume of hemi- or thyroidectomy with or without cervical lymphadenectomy. The selection of patients was continuous. All patients underwent ultrasound or videolaryngoscopy before the operation and on the 1st postoperative day. Patients who underwent thyroidectomy underwent blood analysis for parathormone and blood analysis for ionized calcium.</p></sec><sec><title>RESULTS</title><p>RESULTS: For hemithyroidectomy the duration of surgery did not change. In the group of patients with nodes up to 30 mm the duration of surgery increased. An increase in the operation time was also noted for thyroidectomy. Unilateral laryngeal paresis was registered in 6 (2.3%) patients or 1.7% of the number of recurrent laryngeal nerves in the risk area. The risk of parathyroid gland (PTG) removal was 0.39% of the number of operations and 0.14% of the number of PTG in the risk area. Decrease in the level of parathormone in the first day after surgery was noted in 11.5% of patients.
