На сегодняшний день хирургическое лечение является единственным эффективным способом лечения гиперкортицизма при макронодулярной двусторонней гиперплазии надпочечников (МДГН). Ввиду двустороннего узлового поражения надпочечников наиболее часто в Российской Федерации выполняется двусторонняя адреналэктомия, которая сопряжена с необходимостью пожизненного приема заместительной терапии. Конечные результаты исследований зарубежных авторов по эффективности односторонней адреналэктомии (ОА) при МДГН неоднозначны.</p></sec><sec><title>ЦЕЛЬ</title><p>ЦЕЛЬ. Оценить эффективность ОА у пациентов с МДГН, сопровождающейся функционально-автономной продукцией кортизола (ФАПК) и манифестной формой синдрома Иценко–Кушинга (СК).</p></sec><sec><title>МАТЕРИАЛЫ И МЕТОДЫ</title><p>МАТЕРИАЛЫ И МЕТОДЫ. В условиях хирургического отделения ФГБУ «НМИЦ эндокринологии» ОА проведена 44 пациентам с МДГН и манифестной формой СК (n=21),  а также ФАПК с коморбидными состояниями (n=23). Произведена оценка динамики лабораторных показателей (кортизол в ходе ночного подавляющего теста (ПДТ1), кортизол суточной мочи (СКМ), адренокортикотропный гормон (АКТГ)), а также течения сопутствующих заболеваний (сахарный диабет, артериальная гипертензия (АГ), обменных нарушений) до и через 6 мес после хирургического вмешательства. Определен cut-off уровень кортизола крови (8:00–9:00) в течение первых суток после ОА как предиктора развития надпочечниковой недостаточности (НН) в раннем послеоперационном периоде у пациентов с МДГН.</p></sec><sec><title>РЕЗУЛЬТАТЫ</title><p>РЕЗУЛЬТАТЫ. Ремиссия наблюдалась у 40/44 (91%, 95% доверительный интервал (ДИ) 78–97%) пациентов. Медиана наблюдения — 18 [12; 36] мес. Через 6 мес после ОА статистически значимо изменились лабораторные показатели: уровень кортизола после ПДТ1 снизился с 466 [173; 652] до 86 [61; 149] нмоль/л, р&lt;0,01, СКМ уменьшился с 840 [468; 1892] до 267 [204; 432] нмоль/сут, р&lt;0,01, в то время как показатель АКТГ повысился с 1 [1; 2,1] до 8,3 [2,6; 15,0] пг/мл, р&lt;0,01, что свидетельствует об эффективности ОА. Были получены статистически значимые изменения в ИМТ (до ОА — 30,8 [27,5; 34,4], после — 28,5 [23,9; 32,2] кг/м2; р&lt;0,01). При анализе эффекта ОА на углеводный обмен зафиксировано уменьшение уровня HbA1c с 7,0 [6,3; 7,7] до 6,0 [5,6; 6,7]% (р&lt;0,01). После операции наблюдалось улучшение течения АГ: антигипертензивные препараты были полностью отменены в 27,7% случаев, снижение количества антигипертензивных препаратов потребовалось в 44,4% случаев. Уровень кортизола в раннем послеоперационном периоде менее 325 нмоль/л обладает высокой прогностической ценностью в отношении НН (AUC=0,96).</p></sec><sec><title>ЗАКЛЮЧЕНИЕ</title><p>ЗАКЛЮЧЕНИЕ. ОА может быть рекомендована в качестве первого этапа лечения при манифестной форме МДГН в связи с высокой вероятностью длительной ремиссии гиперкортицизма, улучшения течения коморбидных заболеваний.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>BACKGROUND</title><p>BACKGROUND: To date, surgical treatment is an effective treatment of hypercortisolism in primary bilateral macronodular hyperplasia (PBMAH). Due to the bilateral lesion, the most common treatment in Russia is the bilateral adrenalectomy, which requires the appointment of hormone replacement therapy. The results of various studies on the effectiveness of unilateral adrenalectomy (UA) in PBMAH are rather discordant.</p></sec><sec><title>AIM</title><p>AIM: The objective of the study was to assess the outcomes of UA in patients with PBMAH, accompanied by mild autonomous cortisol excess (MACE) or overt Cushing’s syndrome (CS).</p></sec><sec><title>MATERIALS AND METHODS</title><p>MATERIALS AND METHODS: 44 patients with PBMAH and a manifest form of CS (n=21) MACE with comorbid conditions (n=23) underwent UA in the surgical department of the Endocrinology Research Centre. The dynamics of laboratory parameters (cortisol after overnight dexamethasone suppression test (cortisol ONDST) urinary free cortisol (СКМ), ACTH), as well as the course of comorbid diseases (diabetes mellitus, arterial hypertension (AH), metabolic disorders) were assessed before and after 6 months of surgical intervention. The cut-off level of blood cortisol (8:00–9:00) during the first day after OA was determined as a predictor of the development of adrenal insufficiency (AI) in the early postoperative period.</p></sec><sec><title>RESULTS</title><p>RESULTS: 6 months after OA, laboratory parameters improved in 40/44 (91%, 95% CI: 78%-97%) cases: cortisol ONDST decreased from 466 [173; 652] to 86 [61; 149] nmol/l, p=&lt;0.01, СКМ dropped from 840 [468; 1892] to 267 [204; 432] nmol/day, p=&lt;0.01), while ACTH increased gradually from 1 [1; 2.1] to 8.3 [2.6; 15.0] pg/ml, p=&lt;0.01), which indicates the effectiveness of UA. Statistically significant changes in BMI were also obtained (before UA — 30.8 [27.5; 34.4], after — 28.5 [23.9; 32.2] kg/m2 (p &lt;0.01)). When analyzing the long-term outcomes of UA on carbohydrate metabolism, a decrease in the level of HbA1c from 7.0 [6.3; 7.7] to 6.0 [5.6; 6.7] %, (p&lt;0.01) was observed. Further, there was an improvement in the course of AH. After UA, antihypertensive therapy was completely discontinued in 27.7% of cases, and a reduction in the number of antihypertensive drugs was carried out in 44.4%.
