Феохромоцитома (ФХЦ) — опухоль из хромаффинной ткани мозгового вещества надпочечника, способная к гиперпродукции катехоламинов. Важной особенностью феохромоцитомы, в связи с ее гормональной активностью, является угроза резких изменений гемодинамических показателей во время оперативного лечения. В хирургическом лечении хромаффинных опухолей надпочечников в настоящий момент все большую популярность приобретает ретроперитонеоскопический доступ. В клинической практике применяются два варианта упомянутого доступа: классический (трехпортовый) и однопортовый. В данном исследовании производилась оценка влияния указанных вариантов ретроперитонескопического доступа на интраоперационный контроль гемодинамики.</p></sec><sec><title>ЦЕЛЬ</title><p>ЦЕЛЬ. Сравнение результатов лечения и интраоперационных показателей гемодинамики пациентов с феохромоцитомами, прооперированных с использованием однопортового и трехпортового вариантов ретроперитонескопического доступа.</p></sec><sec><title>МАТЕРИАЛЫ И МЕТОДЫ</title><p>МАТЕРИАЛЫ И МЕТОДЫ. Ретроспективное одноцентровое когортное исследование было проведено на выборке пациентов, первично прооперированных по поводу феохромоцитом в Клинике высоких медицинских технологий им. Н.И. Пирогова СПбГУ с октября 2015 года по февраль 2020-го с интраоперационным инвазивным контролем гемодинамики с помощью систем PiCCO+.</p></sec><sec><title>РЕЗУЛЬТАТЫ</title><p>РЕЗУЛЬТАТЫ. Исследованы 47 пациентов, из которых 24 выполнена однопортовая адреналэктомия (включены в первую группу), 23 — трехпортовая (вторая группа). Средний размер опухоли во второй группе был больше (40,12±9,3 и 56,7±21,5, p&lt;0,05), предположительно, в связи с этим была статистически значимо выше и средняя продолжительность операции (73±21,5 и 111,7±36,1, p&lt;0,05). Индекс массы тела (ИМТ) пациентов в двух группах статистически значимо не отличался. Измеренные интраоперационно показатели максимального и минимального средних систолических и диастолических АД, количество эпизодов повышения диастолического АД выше 120 мм рт.ст. и их продолжительность статистически не различались в обеих группах (p&gt;0,05). Эпизоды повышения систолического АД выше 180 мм рт.ст. во второй группе были чаще (p=0,03), однако их суммарная продолжительность в двух группах была без значимых различий (p&gt;0,05). Интраоперационный расход антигипертензивных препаратов при выделении надпочечника и вазопрессорных препаратов после пережатия центральной вены надпочечника были сходными. АД после пережатия центральной вены в первой и во второй группах значимо не отличалось (p&gt;0,05). Летальных исходов и ­послеоперационных осложнений в обеих группах зарегистрировано не было.</p></sec><sec><title>ЗАКЛЮЧЕНИЕ</title><p>ЗАКЛЮЧЕНИЕ. Однопортовая модификация ретроперитонескопического доступа при наличии достаточного опыта анестезиологической бригады и оперирующего хирурга не приводит к ухудшению контроля гемодинамики и результатов операции, являясь безопасной и эффективной в хирургическом лечении феохромоцитомы.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>BACKGROUND</title><p>BACKGROUND: Pheochromocytoma (PCC) is a tumor from the chromaffin tissue of the adrenal medulla, capable of hyperproduction of catecholamines. An important feature of pheochromocytoma, due to its hormonal activity, is the threat of sudden changes in hemodynamic parameters during surgical treatment. Retroperitoneoscopic access is currently becoming increasingly popular in the surgical treatment of chromaffin tumors of the adrenal glands. In clinical practice, two variants of the mentioned access are used: classic (three-port) and single-port. In this study, the influence of these options of retroperitoneoscopic access on intraoperative hemodynamic control was evaluated.</p></sec><sec><title>AIM</title><p>AIM: Comparison of treatment results and intraoperative hemodynamic parameters of patients with adrenal pheochromocytomas operated with single-port and three-port retroperitonescopic access options.</p></sec><sec><title>MATERIALS AND METHODS</title><p>MATERIALS AND METHODS: A retrospective single-center cohort study was conducted on a sample of patients initially operated for pheochromocytoma at the Pirogov St. Petersburg State University High Medical Technology Clinic from October 2015 to February 2020 with intraoperative invasive hemodynamic control using PiCCO+ systems.</p></sec><sec><title>RESULTS</title><p>RESULTS: 47 patients were examined, 24 of whom underwent single–port adrenalectomy (included in the first group), 23 — three-port (second group). The average tumor size in the second group was larger (40.12±9.3 and 56.7±21.5, p&lt;0.05), presumably, in this regard, the average duration of surgery was statistically significantly higher (73±21.5 and 111.7±36.1, p&lt;0.05). The body mass index (BMI) of patients in the two groups did not differ statistically significantly. Intraoperatively measured values of maximum and minimum mean systolic and diastolic blood pressure, the number of episodes of increased diastolic blood pressure above 120 mmHg and their duration did not differ statistically in both groups (p&gt;0.05). Episodes of increased systolic blood pressure above 180 mm Hg were more frequent in the second group (p=0.03), but their total duration in the two groups was without significant differences (p&gt;0.05). Intraoperative consumption of antihypertensive drugs during the release of the adrenal gland and vasopressor drugs after compression of the central vein of the adrenal gland were similar. Blood pressure after compression of the central vein in the first and second groups did not differ significantly (p&gt;0.05).
