Основной сложностью хирургического лечения первичного гиперпаратиреоза является возможное атипичное расположение околощитовидных желез и множественный характер поражения. Предложено значительное количество методов как для дооперационной, так и для интраоперационной топической диагностики при первичном гиперпаратиреозе, и поиск новых методов продолжается и по настоящее время. Предложенный метод топической диагностики исследования паратгормона из смыва с иглы при пункционной биопсии имеет высокие статистические параметры чувствительности, специфичности и прогностической ценности. Данный метод необходим как дополнительный, уточняющий параметр в топической диагностике первичного гиперпаратиреоза, особенно в случае неоднозначных и сомнительных данных, полученных основными методами топического исследования при первичном гиперпаратиреозе.

The main difficulty of primary hyperparathyroidism in the surgical aspect, it is possible atypical location of the parathyroid glands and multiple nature of the lesion. Proposed significant number of methods for pre- operative and intraoperative for topical diagnosis in primary hyperparathyroidism and the search for new methods and continues to the present. The proposed method of topical diagnostic studies of PTH-FNAB has a high statistical parameters of sensi- tivity, specificity and predictive value. This method is needed as an additional parameter specifying, in the topical diagnosis of primary hyperparathyroidism, especially in the case of ambiguous and questionable data received basic methods of topical studies in primary hyperparathyroidism.
